  • What does it mean to be a homebound patient?

    Most insurance companies will allow medical providers to travel to the place of residence to render care if that patient is homebound. Homebound means the patient has a condition that makes it difficult for them to leave their home. This could include ambulatory challenges that require the use of a wheelchair/walker or conditions such as dementia, COPD, congestive heart failure, or post surgical patients.

  • Will I see a doctor?

    At NNH we are PA owned and operated. We have a collaborating physician that is available for consultation. Your visits will be conducted by the PA who as over 18 years of experience in medicine including specialties such as urgent care, occupational health, cardiology, nephrology, and geriatrics.

  • Do you offer telehealth appointments outside of regular business hours?

    NNH has a provider that will make any and all effort to be available for telehealth appointments outside of Mon-Friday 8-5pm. My goal is to reduce your access barriers and allow you to be seen quickly and efficiently. I know that the health needs of ourselves and our families do not occur always during normal business hours. At NNH I want to make sure we are available to our patients when they need us.

  • Do you offer any lab tests?

    At NNH we offer some point of care tests that we complete on site including strep throat screening, rapid influenza, and urine dipsticks for urinary tract infections.

    If you are in need of additional tests NNH will give you a order you can take to your local lab or hospital to obtain.

  • Do you offer imaging services?

    Our homebound patients have access to our mobile imaging unit. Through our contract we are able to over mobile on site x-rays, ultrasounds, and ECGs.

    For patients who are not homebound we can send a order to your local hospital for you to have your necessary imaging completed.

  • What is your service area?

    NNH currently sees homebound patients in Waterloo, Cambridge, Jefferson, Johnson Creek, Fort Atkinson, Watertown, and Oconomowoc.

    Telehealth appointments can be done for ANYONE residing in Wisconsin. You do not need to live in our service area!
